GENERAL CONDUCT

• Once you get into the premises, please take a seat at the waiting hall and wait to be called in.
• Is a Must to wear a face covering when you get into the premises. Please use the hand sanitizer provided at the reception area before and after treatments.
• You should not book the appointments if you have any symptoms of COVID19 or any Family members are diagnosed recently, please stay at home and look after yourself until you fully recover, we will be happy to reschedule your appointment by phone.
• All clients must complete a consultation form prior to treatment and will receive a brief consultation/feedback as part of their treatment.
• All treatments are for therapeutic purposes only.
• Treatments will only be conducted if the therapist deems the treatment safe for the client to receive.
• Immediate termination of the session will ensue if the customer engages in illicit or sexually suggestive remarks, sexual advances, drug use, excessive drinking and/or other inappropriate behaviour, racist behaviour.
• If the session is terminated due to the customer’s inappropriate behaviour, the customer is responsible for payment in full and his/her credit charge will be processed.

CANCELLATION POLICY

• Cancellation is expected 24 hours in advance for all clients. If you provide less than 24 hour notice or are a “No-Call, No-Show,” you will be responsible for a full fee payment. Cancellation fee must be paid within 10 days of the cancellation.
• Sorry, but we don´t do refunds, however if you do the cancellation with at least 24 hrs notification, you can reschedule your appointment and use your deposit (whit no expired date) for next appointment.
• If you are late, you will receive only the amount of time remaining of your scheduled appointment. The massage therapist has the right to charge the client in FULL for said services.
• Chi balance Therapies has the right to cancel a treatment without notice if the client is intoxicated or inebriated.

PRIVACY

• Client treatment details or consultations will not be discussed with anyone other than the client unless the client is under the age of 18 or has a care worker or guardian.
• Consultations will only be discussed with the client unless the client is under the age of 18 or has a care worker or guardian.
• Every client must complete an intake form prior to receiving treatment and all information provided will be kept under strictly professional confidentiality and in a secured file.
• By signing the Intake forms, clients are giving Chi Balance Therapies permission to hold records and data concerning the person.
• No photography/ video inside the treatments room.

CLIENTS WHO ARE MINORS

• Clients under 16 years of age must be accompanied by an adult.
• The adult must give permission for the treatment to be performed, along with co-signing the Intake form.
• The adult must be present inside the therapy room if the minor is under 12 and /or wait outside the room if the consultant is older, in this case should provide with an extra contact number in case of emergency.

I UNDERSTAND AND AGREE TO THE FOLLOWING:

• No one of the treatments provided by Chi Balance Therapies are not a substitute for a medical examination or for treatment. I am responsible for seeking the advice of a physician, chiropractor or other qualified medical specialist for any mental and/or physical ailments. I will disclose to the holistic therapist all known medical conditions before the treatment have been done. I will keep the provider updated as to any changes in my medical profile. I understand that there will be no liability on the part of Chi Balance Therapies if I fail to disclose any new or updated medical issues.
• I am at least 18 years old and I am booking an appointment for myself or someone else who is at least 18 years old. As an adult, I understand my responsibilities as well as the risks as described in the terms and conditions.
• All appointments must be made directly through the Chi Balance Therapies website by using the “BOOKING” button or contacting the therapist by the contact number provided in Contact Us section.
• Chi Balance Therapies is not responsible for any personal injury or property loss
• Receipts for Laya and other health insurances will be sent to clients after booking the appointment.
• For those who cancel appointments (See Cancellation Policy).
• Chi Balance Therapies has the right to refuse service at any time without recourse.
• Chi Balance Therapies reserves the right, at our sole discretion, to change, modify or otherwise alter these terms and conditions at any time.
